VA Forms

This is a short list of some of the most common required forms the VA requests for applying for VA benefits or appealing decisions. These tabs will take you to the official VA website, so you have access to the most current forms. If you need assistance filing the PDF versions of the forms please request assistance. Many of these can be completed online.
The Intent to File is the first step you should take when starting your initial claims process. Submitting an intent to file can secure the earliest possible effective date for any retroactive payments you may be eligible to receive on any new issues. You have 1 year from filing this request to submit the New Claim form.
New Claim Forms are for any disabilities that have not been filed previously. 

This form is for re-opening previously denied issues that have been closed out after the 1 Year appeal period. (1 Year after the date on the official Rating Decision Letter)

This form is for a denied issue that you want to have re-evaluated. It is used to keep a claim open on appeal, in order to keep the earliest effective date possible. You will need to submit new evidence, not already provided to the VA previously. 

This form is for denied issues that you have already submitted all the evidence and you believe the Rater made an error in the proper rating based on given information. It is best to speak with your representative before filing, as you will want an informal call with the Rater to discuss the errors.

This form is used for Veterans who are unable to earn over the national poverty threshold due to their VA disabilities. You must have an overall rating of at least 70% with the main disability being at least 40% or higher. Please contact me for more details prior to submitting.

    This organization has great information on PFAS chemicals and the health conditions linked to them. It is beneficial to check out the map and see if you were stationed at any of the bases, if you have any of the conditions listed.
